Referrals and AdmissionsNo one is more vulnerable than the medically fragile individual who is dependent upon caregivers for their every need. Families must be careful and selective when choosing a facility to provide this precious care. The Hallmark Development Center, which provides long and short-term care for these individuals, is one of four skilled nursing care facilities in Massachusetts, and the only facility serving western Massachusetts. The Hallmark programs provide a continuum of care for each individual’s medical, social, and educational development. We work with family members, doctors, discharge planners, and local school systems to provide a smooth and gentle transition whether it is for short or long-term care. The Massachusetts Department of Public Health requires medically complex children to complete a screening by the Medical Review Team (MRT) prior to being placed in a pediatric nursing care facility. Placements are generally limited to individuals meeting the following criteria:
The Medical Review Team also considers placement of children and young adults who are in need of short-term rehabilitation due to recent hospitalizations or acquired brain injuries.
